Thank you for your interest in becoming an Adult/Child Developmental Home Provider (ADH/CDH). We provide equal opportunities for all vendor applicants based on their demonstrated abilities, experience, and training.

We are contracted with the State of Arizona and most of our requirements are based on their mandates for being a certified provider.

Please note – Matching providers with the family is an important and personal process.  Achieve, Inc.  strives to deliver services as soon as possible and providing the proper match may take up to a month.

Questions & Answers

WHAT IS A DEVELOPMENTAL HOME PROVIDER?  

A Developmental Home Provider is a dedicated family or individual who actively opens their home and their hearts to adults or children with intellectual and developmental disabilities. These providers empower individuals to thrive within their communities by providing essential support. We are committed to achieving this mission by collaborating with individuals and families who demonstrate integrity and consistency, ensuring a safe and nurturing environment where everyone can flourish.

 

WOULD THE PERSON LIVING WITH ME HAVE FAMILY MEMBERS INVOLVED IN THEIR LIFE?

Absolutely. Many individuals we serve have family and friends who actively participate in their lives. For those who do not, we will ensure you receive the support needed to engage with any involved family members until you feel at ease.

 

WHAT HAPPENS IF I HAVE A DUI ON MY RECORD?

If the DUI is a misdemeanor that occurred less than 5 years prior to your application for a Clearance Card, be aware that a driving restriction will be placed on the card. This means you will be prohibited from transporting children or vulnerable adults.

 

WHAT IS THE CRIMINAL HISTORY SELF-DISCLOSURE AFFIDAVIT?

The Affidavit is a sworn statement where you disclose any convictions or pending indictments. It serves as a vital pre-screening tool to assess your suitability for working with children or vulnerable adults. You will complete this Affidavit before fingerprinting, and it must be notarized.

 

WILL I NEED TO PASS A CRIMINAL BACKGROUND CHECK?

Absolutely. Anyone contracting with DDD to provide services to children or adults with special needs must possess a valid Level One Fingerprint Clearance Card and undergo a background check through the Department of Child Safety.

 

WHO CAN BECOME A DEVELOPMENTAL HOME PROVIDER?

Providers come from diverse backgrounds and skill levels, including stay-at-home moms, retirees seeking additional income, empty nesters desiring companionship and a way to give back, and seasoned social services professionals experienced in caring for individuals with intellectual and developmental disabilities. Our providers may be single or married, men or women, representing various ethnicities and religions. What unites all our home providers is their unwavering commitment to supporting individuals in their care, helping them grow, thrive, and successfully integrate into the community.

 

WHAT DO DEVELOPMENTAL HOME PROVIDERS DO?  

Developmental Home Providers deliver personalized services that align with the unique desires and needs of the individuals residing with them. The level of support, supervision, and active assistance is tailored to each person's specific requirements. Our thorough matching process takes into account the abilities, strengths, needs, desires, and preferences of both the provider and the individual, ensuring a successful partnership.

 

WHAT ARE PROVIDERS, AND DO THEY EARN MONEY FOR SERVICES PROVIDED IN THEIR HOMES?

Adult Developmental Home (ADH) and Child Developmental Home (CDH) providers are licensed independent contractors who create nurturing family environments. They play a crucial role in offering positive emotional support to individuals with intellectual and developmental disabilities while equipping them with essential skills for fulfilling and successful lives.

These dedicated providers receive per diem compensation for Room and Board (covering household expenses) as well as for Habilitation (the support and training they provide). Each individual has an "Individual Service Plan" (ISP) that the provider reinforces, ensuring a structured and effective approach to care. Best of all, the compensation earned is TAX-FREE! Providers are empowered to manage their own businesses, enhance their professional skills, and maintain the necessary insurance, licensing, and certifications. They adhere to all federal, state, or county regulatory requirements, reinforcing their commitment to providing high-quality support.

 

WHAT KINDS OF CARE WILL I BE RESPONSIBLE FOR?

 As a provider, you will confidently take on the responsibility of delivering essential daily living care and related tasks. This includes preparing nutritious meals, offering tailored supervision based on the specific needs of the individual in your home, providing transportation to appointments and recreational activities, and facilitating access to community events. Moreover, home providers are expected to maintain thorough records and documentation of the services and supports provided. Our Team will collaborate with you to equip you with all the necessary information and materials to ensure your success in welcoming an individual into your home.

 

WILL FRIENDS AND FAMILY OF THE PERSON I SUPPORT BE ABLE TO "DROP BY" UNANNOUNCED?

 Just as your loved ones may visit without prior notification, the family, and friends of the person you support, as well as your coordinator, may also stop by unexpectedly. While this should not be a frequent occurrence, if unplanned visits become an issue, our Team is ready to help you find a practical solution that works for everyone involved.

 

HOW MANY INDIVIDUALS CAN I (OR MY FAMILY) HAVE LIVING WITH ME (US) AT ONE TIME?

Each provider has the flexibility to determine whether one, two, or three individuals is the right fit for them, within the guidelines established by the State of Arizona, which allows for a maximum of three residents. Typically, home providers serve one or two individuals, ensuring that each person has their own private bedroom.

 

HOW LONG IS THE COMMITMENT TO HAVE THE PERSON LIVE WITH ME? 

Our comprehensive matching process is designed to create meaningful connections between you and the individual who will join your home. Some of the most successful placements have flourished for decades, while others may last a few years as needs evolve. Many former providers maintain lifelong friendships with the individuals they supported.

 

WILL THE PERSON WHO LIVES WITH ME NEED CONSTANT SUPERVISION? 

The individuals we serve have diverse needs; some require constant supervision while others are more independent. Our dedicated team will ensure that you are paired with someone whose needs align perfectly with your lifestyle, guaranteeing a harmonious living situation.

WHO DO I CALL IF I HAVE AN ISSUE OR NEED HELP?

At Kompton House, LLC, we are dedicated to supporting you and the individual you care for. You are never alone on this journey. Each home is assigned a skilled coordinator who provides essential case management services. Our coordinators are experienced human service professionals who oversee the individual's daily progress. They ensure that all needs are met, license standards are upheld, and that the individual's service plan (ISP) is actively followed. Expect regular visits from the coordinator to both you and the Home Provider.

 

WOULD I BE MATCHED WITH A PERSON OF THE SAME OR OPPOSITE GENDER?

Our matching process is designed specifically to ensure both you and the individual who joins your home feel comfortable and can coexist harmoniously as a family. You have the final say over who is welcomed into your home, prioritizing both comfort and compatibility.

 

HOW LONG DOES THE MATCHING PROCESS TAKE? 

The matching process kicks off once you are licensed and ready to welcome someone into your home. This process is partly dependent on your availability and can take anywhere from a few weeks to several months. We believe that taking the time for proper matching is crucial and leads to creating lasting, meaningful relationships. Many of our matches have transformed into long-term partnerships built on trust and understanding.

What kinds of disabilities will the person I support have?

In a Developmental Home Provider setting, individuals with a range of developmental disabilities receive services. This includes those with intellectual disabilities, autism, cerebral palsy, and epilepsy. Some individuals are quite independent and can engage with the community for short periods, and some may even hold jobs. Others may require more intensive support. Rest assured, our team will carefully match you with a person whose needs align with your lifestyle.

 

DO I GET RESPITE?

We fully recognize that our licensed providers need respite occasionally. To provide maximum flexibility, our program includes respite funding in the provider's monthly reimbursement. This empowers our licensed providers to collaborate with respite providers that suit the needs of both the members and their families. Licensed providers will directly pay respite providers at a mutually agreed-upon rate. It is imperative that all respite providers are approved by the legal guardian, DDD support coordinator, and Kompton House, LLC support specialist before services begin. Additionally, it is the licensed providers' responsibility to submit all required documentation before utilizing respite services. If a respite provider is not secured, the Kompton House, LLC support staff is here to help you explore options and establish relationships with respite providers, ensuring continuity of care during respite periods. Who oversees the management of an individual's finances and personal belongings?

Kompton House, LLC, alongside the Division of Developmental Disabilities (DDD), establishes clear guidelines to support individuals in managing their personal spending and property effectively. Your coordinator will partner with you to implement the right financial procedures and set up necessary accounts.

 

WHAT SHOULD I BE PROVIDING FOR THIS INDIVIDUAL?

A Provider is firmly responsible for delivering nutritious meals and snacks, as well as essential personal care items and hygiene supplies, including shampoo, soap, and toothpaste. Clothing, footwear, bedding, and furnishings for the bedroom will be purchased using the individual's personal spending money. Providers, on the other hand, must cover their own housing costs, utilities, cell or landline phone services, and cable expenses. The individual will manage their personal cell phone or TV connection.

 

AM I ACCOUNTABLE FOR TRANSPORTATION?

The Provider is fully responsible for transportation arrangements to medical appointments, employment or day program activities, recreational outings, and other social engagements.

What if the person who lives with me must leave later or get home earlier than I do?

During the matching process, the coordinator considers the provider's and the individual's daily schedules and routines. Many primary home providers benefit from having a secondary home provider who is trained and certified to provide support and back-up when you have other commitments.

 

CAN I STILL WORK WHILE BEING A PROVIDER?

Home providers can have a part-time or full-time job away from their provider duties. Our Team will answer any questions about this arrangement. Some Providers will drop their individual off at their employment or day program site on their way to work and pick them up after work.

WHAT HAPPENS IF THE PERSON I AM LIVING WITH NEEDS TO LEAVE BEFORE I DO OR ARRIVES HOME EARLIER?

During the matching process, our coordinator takes into account both the provider's and the individual’s daily schedules and routines. Many primary home providers find it beneficial to have a secondary home provider who is fully trained and certified. This ensures reliable support and backup for those times when you have other commitments.

 

WHAT WILL THE INDIVIDUAL DO DURING THE DAY?

Most individuals placed in a Provider home are engaged in jobs, work training, or day programs for 30-40 hours a week. There may be occasions when a specific individual may not be suited for work or day programs, but rest assured, your coordinator will discuss this thoroughly before any match meetings occur. We are committed to collaborating with you and your family to ensure that the needs, desires, and preferences of the individual living with you align seamlessly with your lifestyle.

 

WHAT IS INVOLVED WITH LICENSE RENEWAL?

For license renewal, providers must complete 10 hours of continuing education training each year, along with updated certification in CPR, First Aid, Article 9 (Managing Inappropriate Behaviors), and Prevention and Support, if applicable. Fingerprinting, physician statements, and home inspections are required every three years. Alongside the training and certifications, a reapplication packet must be submitted. Your Licensing Support Specialist will simplify this process, guiding you through the reapplication steps and paperwork. Expect a renewal letter approximately three months before your license expires, detailing the required procedures and verifying home compliance with Licensing Regulations.

 

WHAT DOES THE DEVELOPMENTAL HOME PROGRAM TEAM DO?

The Developmental Home Program Team is pivotal in supporting providers in various roles, including Licensing Worker, Home Supervisor, DDD Liaison, Training Coordinator, and Support Specialist for assigned homes. As your Licensing Support Specialist, I ensure homes meet licensing regulation compliance while providing technical assistance for any emerging needs. We conduct yearly license renewal assessments to uphold license status and facilitate valuable training opportunities for providers. Regular monthly home visits and frequent phone interactions are established to maintain a strong relationship with each provider and ensure adherence to Licensing Regulations in line with the Arizona Administrative Code, Title 6, Chapter 6, Articles 10 and 11.

We monitor compliance with Agency Standards for Developmental Home Service Provision and advocate for individuals placed in our homes through visits to their homes, schools, and vocational settings. We also engage in enjoyable leisure activities with our families and individuals. Our team is dedicated to assisting Providers with monthly paperwork for each individual, through pre-service training and clearly defined due dates. One of our primary objectives is to facilitate and strengthen relationships between Providers and other service Team members (such as Case Managers and Therapists) to ensure the necessary services and support are in place for individuals to lead fulfilling lives. We proudly champion independence through family homes!

As a Licensing Support Specialist, we continually monitor homes for licensing regulation compliance and offer technical assistance when areas of need emerge. We complete yearly license renewal studies to maintain license status and facilitate Provider training opportunities. Monthly home visits are made, and frequent phone contact occurs to keep our relationship with each provider and ensure compliance with Licensing Regulations, according to the Arizona Administrative Code, Title 6, Chapter 6, Articles 10 and 11. We also monitor for Agency Standards for Developmental Home Service Provision. Your Licensing Support Specialist also works as a client/consumer advocate for individuals placed within our homes through visits to home, school, day, and vocational settings. We occasionally enjoy leisure pursuits with our families and individuals. We assist our Providers with monthly paperwork completion for each individual through pre-service training and set guidelines for due dates. One of the most essential things the Team does is assist Providers and Consumers to maintain a relationship with other service Team members (Case Managers, Therapists, etc.) by acting as a guide or liaison to get the services and support needed to live a fulfilling life. We proudly foster independence through family homes!
